Welcome to PureForma. These Terms & Conditions (“Terms”) govern your access to and use of our website, online store, products, services, programs, events, and related offerings.

For purposes of these Terms, “PureForma,” “PF Wellness,” “we,” “us,” and “our” refer to PF Wellness and the brands and offerings within the PF Wellness family, including PureForma Labz and Aurevon, as applicable.

By accessing this website, creating an account, placing an order, booking a service, enrolling in a program, or otherwise using our website or services, you agree to these Terms.

If you do not agree to these Terms, do not use the website or purchase our products or services.
 

1. AGE REQUIREMENT

This website is intended for individuals 21 years of age or older.

By accessing the website or placing an order, you represent that you are at least 21 years old and legally capable of entering into a binding agreement.

Certain products, services, events, or programs may have additional eligibility requirements.
 

2. THE PF WELLNESS FAMILY

PF Wellness encompasses multiple categories of products and services. Different terms, restrictions, and disclaimers apply depending on the offering.

These may include:

PureForma Labz Research Products
Laboratory and research materials intended exclusively for lawful research purposes.

Aurevon
Consumer skincare and aesthetics products offered under the Aurevon brand.

Biomarker Testing
Access to selected biomarker testing panels and laboratory services performed through independent third-party laboratories.

Consultations and Educational Services
Informational and educational sessions related to research, laboratory data, products, and available services.

Cryotherapy and Other Wellness Services
In-person services that may require separate eligibility screening, acknowledgments, waivers, or informed-consent documentation.

Events, Affiliate Programs, Referral Programs, and Wholesale Programs
Separate rules or agreements may apply to these activities.

A disclaimer or restriction applicable to one category should not be interpreted as applying to a different category unless expressly stated.

3. PUREFORMA LABZ RESEARCH PRODUCTS

Products identified as Research Use Only, RUO, or otherwise sold through the PureForma Labz research catalog are intended strictly and exclusively for lawful laboratory and research purposes.

These products are:

FOR RESEARCH USE ONLY. NOT FOR HUMAN OR VETERINARY USE.

Research products are not sold or represented as medications, compounded medications, medical treatments, veterinary products, foods, dietary supplements, or consumer cosmetic products.

They are not intended for human or animal consumption or administration.

Research products may not be used for:

• Human consumption or administration

• Injection or other administration to a human or animal

• Veterinary use

• Diagnosis, treatment, cure, mitigation, or prevention of any disease or medical condition

• Patient management or clinical treatment decisions

• Food or dietary supplement use

• Consumer cosmetic use

• Any purpose inconsistent with the product's research-use designation

By purchasing a research product, you affirm that you understand and accept these restrictions and that your purchase is solely for a lawful research purpose.
 

4. PURCHASER RESPONSIBILITY FOR RESEARCH MATERIALS

Purchasers are solely responsible for determining whether they possess the knowledge, facilities, equipment, procedures, and authority necessary to receive, handle, store, use, transfer, and dispose of research materials safely and lawfully.

You are responsible for compliance with all federal, state, local, institutional, and other laws or regulations applicable to your purchase and research activities.

PureForma reserves the right to refuse, cancel, limit, or restrict any research-product order when we reasonably believe that the purchaser intends to use a product for an unauthorized purpose or has otherwise violated these Terms.

We may also request additional confirmation regarding intended research use before fulfilling an order.
 

5. RESEARCH PRODUCT INFORMATION

Information presented on the PureForma Labz website, including product descriptions, references to scientific literature, research pathways, compounds, mechanisms, biomarkers, or published studies, is provided solely for general educational and research-reference purposes.

Such information does not constitute instructions for human or veterinary use.

PureForma does not provide dosing protocols, administration instructions, treatment recommendations, prescribing advice, or individualized medical recommendations for research products.

Discussion of a compound in published scientific research does not mean that a PureForma Labz research product is approved or intended for the same use.

6. THIRD-PARTY TESTING AND PRODUCT INFORMATION

Certain PureForma Labz research products may be independently tested by third-party laboratories.

Certificates of analysis, analytical reports, purity information, and other testing information are provided for research and quality-reference purposes and may be specific to a particular lot or batch.

Third-party testing does not convert a research product into a medication, medical product, veterinary product, dietary supplement, or product approved for human use.

While we work to provide accurate product information, analytical methods, tolerances, reporting standards, and results may vary between independent laboratories.

7. NO MEDICAL ADVICE

Nothing appearing on this website should be interpreted as medical advice, diagnosis, prescribing, treatment, or the establishment of a physician-patient or other healthcare-provider relationship.

Website content, educational materials, consultations, research information, biomarker information, product discussions, social-media content, and event presentations are provided for informational or educational purposes within the context in which they are offered.

Questions regarding personal medical conditions, medications, diagnoses, treatment, or healthcare decisions should be directed to an appropriately licensed healthcare professional.

8. BIOMARKER TESTING

PF Wellness may provide access to biomarker testing panels designed to provide laboratory information relating to selected biomarkers.

Depending on the testing option, laboratory testing may be performed by independent third-party laboratories, including nationally or regionally available laboratory providers.

PureForma does not control all aspects of third-party laboratory operations, including appointment availability, specimen collection procedures, laboratory processing, turnaround times, laboratory methodology, reference ranges, reporting systems, or temporary service interruptions.

Biomarker panel descriptions and educational information provided by PureForma are intended to help customers understand available testing options and the types of biomarkers included.

They are not intended to diagnose, treat, cure, mitigate, or prevent disease.

Laboratory results should not be used as a substitute for professional medical evaluation. Individuals seeking clinical interpretation, diagnosis, or treatment should consult a qualified healthcare professional.

Third-party laboratories may impose their own terms, privacy practices, eligibility requirements, collection requirements, or other conditions.

9. CONSULTATIONS AND EDUCATIONAL SERVICES

PureForma may offer telephone, virtual, in-person, or other educational consultations.

Unless expressly stated otherwise, PureForma consultations are informational and educational services and are not medical appointments.

Consultations do not include medical diagnosis, prescribing, individualized medical treatment, or the establishment of a physician-patient relationship.

Appointment pricing and scheduling requirements are shown at the time of booking and may change.

Customers must follow the cancellation, rescheduling, lateness, and missed-call policies disclosed during booking. PureForma may consider an appointment missed when the customer cannot be reached after the number of contact attempts specified in the applicable appointment policy.

Any store credit associated with a consultation is subject to the terms disclosed when the consultation is purchased and has no cash value unless required by law.

10. AUREVON AND OTHER CONSUMER PRODUCTS

Products sold under Aurevon or otherwise expressly identified as consumer skincare, cosmetic, personal-care, or wellness products are not PureForma Labz research-use products.

The Research Use Only restrictions in these Terms do not apply to products expressly marketed and labeled for consumer cosmetic or personal-care use.

Consumer products must be used only according to the instructions, warnings, storage requirements, and intended use stated on their packaging or product page.

Product ingredients should be reviewed before use. Customers are responsible for determining whether a product is appropriate for them and should discontinue use if an unexpected reaction occurs.

Unless expressly stated otherwise, cosmetic and skincare products are not intended to diagnose, treat, cure, or prevent disease.

11. CRYOTHERAPY AND IN-PERSON WELLNESS SERVICES

PF Wellness may offer cryotherapy or other in-person wellness services.

These services may be subject to separate eligibility requirements, health and safety screening, instructions, acknowledgments, informed-consent forms, releases, or waivers.

Any service-specific consent or waiver forms are incorporated into these Terms by reference with respect to that service.

Participation may be refused or discontinued when staff determine that applicable safety, eligibility, or operational requirements have not been satisfied.

Information about wellness services is not a substitute for professional medical advice.

12. PRODUCT AVAILABILITY AND WEBSITE INFORMATION

We make reasonable efforts to keep product descriptions, images, inventory, pricing, testing information, and availability accurate.

However, mistakes may occur.

PureForma reserves the right to correct typographical errors, pricing errors, product descriptions, availability information, promotional information, or other website content at any time.

Product packaging, appearance, labeling, suppliers, analytical results, availability, and specifications may change without notice where legally permitted.

Website images may be representative and may not exactly reflect the product delivered.

13. ORDERS

Submission of an order does not constitute final acceptance by PureForma.

We reserve the right to accept, decline, limit, hold, verify, or cancel an order for reasons including inventory availability, pricing or website errors, payment issues, suspected fraud, suspected unauthorized product use, compliance concerns, shipping restrictions, or violation of these Terms.

We may limit quantities or restrict purchasing when reasonably necessary.

An automated order acknowledgment does not guarantee that an order has been accepted or will be fulfilled.

14. PAYMENT

Available payment methods and payment instructions are provided during or following the checkout process.

Current payment options may include third-party payment services such as PayPal, Venmo, Apple Cash, and Zelle, although available methods may change.

Where payment instructions are provided after an order is submitted, payment is generally required within two hours unless another timeframe is stated.

Orders that remain unpaid beyond the applicable payment window may be canceled and inventory released.

An order is not considered paid until payment has been received and verified.

Customers are responsible for including any requested order number or identifying information with payment.

Payments are also subject to any applicable terms imposed by the third-party payment provider.

15. SHIPPING

Shipping options, rates, thresholds, carrier availability, and estimated delivery times are displayed on the website or communicated during checkout.

UPS is PureForma's primary shipping carrier. Certain preorders, special shipments, or other order types may be fulfilled through another disclosed service, including USPS Ground Advantage or UPS Ground Saver.

Orders containing both immediately available and preorder items may be shipped separately.

Shipping and delivery estimates are estimates only and are not guarantees.

Carrier delays, weather, transportation interruptions, holidays, incorrect addresses, access restrictions, and circumstances outside PureForma's control may affect delivery.

Customers are responsible for providing a complete and accurate delivery address.

16. SAME-DAY SHIPPING

When same-day shipping is offered, stated cutoff times refer to the time by which payment must be received and confirmed, not merely the time an order is submitted.

PureForma's current same-business-day shipping target generally requires confirmed payment by approximately 1:45 PM Pacific Time, with carrier pickup cutoff occurring shortly thereafter.

Orders or payments received after the applicable cutoff may ship the next business day.

Same-day shipment is a processing target and is not a guarantee of carrier delivery time.

17. LOCAL PICKUP/DELIVERY

Local pickup/delivery may be available within selected service areas, including qualifying locations within the 209 area.

Eligibility, delivery timing, order deadlines, fees, minimum purchases, and available delivery days may vary and will be stated when the service is offered.

Customers must provide accurate delivery information and ensure that the delivery location can safely and lawfully receive the order.

18. RETURNS, REFUNDS, DAMAGES, AND ORDER ISSUES

Different categories of products and services may have different cancellation, return, refund, or replacement requirements due to the nature of the item or service.

The return or refund policy displayed on the applicable product, service, booking page, checkout page, or separate Refund Policy is incorporated into these Terms.

Certain items may be final sale where disclosed and permitted by law.

Customers should inspect shipments promptly and contact PureForma as soon as possible regarding an incorrect item, missing item, shipping damage, or other fulfillment problem.

Photographs, packaging, shipment information, or other documentation may be required to evaluate a claim.

Nothing in these Terms limits any non-waivable consumer right provided by applicable law.

19. PROMOTIONS, DISCOUNTS, AND STORE CREDIT

Promotional codes, affiliate codes, referral offers, bundles, store credits, gift cards, discounts, giveaways, and other offers are subject to their stated terms.

Unless expressly permitted:

Promotions may not be combined, stacked, duplicated, transferred, reproduced, manipulated, or applied retroactively.

PureForma may correct or cancel a discount that was applied because of a technical error, unauthorized distribution, misuse, or violation of the promotion's terms.

Store credit and promotional credit have no cash value and are not redeemable for cash except where required by law.

Promotion dates and deadlines are based on Pacific Time unless otherwise stated.

20. AFFILIATE AND REFERRAL PROGRAMS

Participation in a PureForma affiliate or referral program is subject to the separate terms of that program.

Affiliate participants may not make unauthorized medical, treatment, disease, diagnostic, veterinary, dosing, human-use, guaranteed-result, or misleading claims regarding PureForma Labz research products.

Affiliates are responsible for complying with advertising, endorsement, disclosure, social-media, and other laws applicable to their promotional activity.

Commission eligibility, attribution, customer ownership, payout timing, inactivity requirements, qualifying sales, excluded transactions, promotional-code use, and termination requirements are governed by the applicable Affiliate Program terms.

If these Terms conflict with a signed or accepted Affiliate Agreement regarding affiliate activity, the Affiliate Agreement controls.

21. WHOLESALE AND BUSINESS-TO-BUSINESS SALES

Wholesale, reseller, authorized-partner, and other business-to-business purchases may be subject to separate pricing, order minimums, payment requirements, fulfillment terms, resale restrictions, research-use requirements, claims restrictions, and agreements.

Unless expressly permitted, wholesale pricing may not be combined with retail coupons, affiliate offers, referral discounts, promotional discounts, or other incentives.

Wholesale and reseller purchasers are independently responsible for the lawful storage, handling, resale, advertising, and distribution of products purchased from PureForma.

If a separate wholesale or partner agreement conflicts with these Terms regarding a wholesale transaction, the applicable wholesale or partner agreement controls.

22. EVENTS, GIVEAWAYS, AND PRIZE PROMOTIONS

PureForma may host educational, networking, promotional, or community events.

Attendance at an event does not create a healthcare-provider relationship.

Research products, research discussions, games, drawings, or other research-specific activities may be restricted to participants 21 years of age or older.

Giveaways, drawings, contests, prize wheels, sweepstakes, or similar promotions may be governed by separate official rules. Those rules control if they conflict with these general Terms.

No purchase is necessary where required by applicable law.
 

23. THIRD-PARTY SERVICES

The website may interact with or provide access to third-party companies, including laboratories, payment services, shipping carriers, appointment systems, website platforms, social-media services, and other vendors.

PureForma is not responsible for the independent acts, omissions, systems, policies, outages, privacy practices, or performance of third parties outside PureForma's reasonable control.

Your use of a third-party service may also be governed by that provider's own terms.

24. ACCOUNTS AND WEBSITE SECURITY

If you create an account, you are responsible for maintaining the confidentiality of your login information and for activity occurring through your account.

You may not impersonate another person, provide knowingly false information, attempt to gain unauthorized access to the website, interfere with website operation, introduce malicious software, scrape or harvest information unlawfully, or use the website for fraudulent or unlawful purposes.

PureForma may suspend or terminate website access when reasonably necessary to protect the business, customers, website, or third parties.

25. INTELLECTUAL PROPERTY

The PureForma Labz, PF Wellness, and Aurevon names, logos, designs, packaging, photographs, graphics, product imagery, written content, educational materials, mascots, marketing materials, and other original content are owned by or licensed to PureForma unless otherwise indicated.

Website content may not be copied, reproduced, republished, distributed, modified, sold, or commercially exploited without prior written permission, except as permitted by law.

Participation in an affiliate or wholesale program does not transfer ownership of PureForma intellectual property.

26. REVIEWS, PHOTOS, AND USER-SUBMITTED CONTENT

If you voluntarily submit a review, photograph, testimonial, comment, or other content to PureForma, you represent that you have the right to submit it and that it does not violate another person's rights.

PureForma may moderate or decline content that contains unlawful claims, medical or dosing recommendations involving research products, misleading statements, offensive material, confidential information, or content inconsistent with our research-use policies.

PureForma is not obligated to publish every submission.

27. PROHIBITED USE

You may not use the website, products, services, programs, or content:

For an unlawful purpose;

To encourage unlawful activity;

To misuse PureForma Labz research products for human or veterinary purposes;

To obtain research products with the intent to administer them to a human or animal;

To misrepresent PureForma products as medications or approved treatments;

To make unauthorized medical, disease, treatment, dosing, or guaranteed-result claims on PureForma's behalf;

To commit fraud or payment abuse;

To interfere with website security;

To infringe intellectual-property rights; or

To violate these Terms or applicable law.

28. DISCLAIMER OF WARRANTIES

To the maximum extent permitted by law, the website and its content are provided on an “as available” basis.

Except for warranties expressly provided in writing or warranties that cannot legally be excluded, PureForma makes no additional express or implied warranty regarding uninterrupted website availability, third-party services, shipping-carrier performance, laboratory turnaround times, or outcomes associated with misuse of products or services outside their stated intended purpose.

Nothing in this section eliminates rights that cannot legally be waived.

29. LIMITATION OF LIABILITY

To the maximum extent permitted by applicable law, PureForma and its owners, employees, contractors, representatives, and affiliates will not be liable for indirect, incidental, special, exemplary, punitive, or consequential damages arising from use of the website, products, services, or third-party providers.

PureForma is not responsible for injury, loss, damage, or liability resulting from the intentional or unauthorized human or veterinary use of a product clearly sold and labeled for research use only.

These limitations apply only to the extent permitted by law and do not exclude liability that legally cannot be limited or excluded.

30. INDEMNIFICATION

To the extent permitted by law, you agree to defend, indemnify, and hold harmless PureForma and its owners, employees, contractors, representatives, and affiliates from claims, damages, losses, or expenses arising from your unlawful use of the website, violation of these Terms, unauthorized resale or marketing activity, infringement of another person's rights, or misuse of a PureForma Labz research product outside its stated research-only purpose.
 

31. PRIVACY

Use of the website is also subject to our Privacy Policy.

The Privacy Policy explains how information may be collected, used, stored, shared, and protected.

Because certain services, including laboratory testing, may involve information handled by independent third parties, those parties may maintain separate privacy notices and practices.

32. ELECTRONIC COMMUNICATIONS

By submitting an order, booking a service, registering for a program, or otherwise providing contact information, you consent to receive transactional communications necessary to complete that activity, including payment instructions, order updates, shipping information, appointment notices, and service communications.

Marketing communications are governed by applicable law and any consent or opt-out choices associated with those communications.
 

33. CHANGES TO THESE TERMS

PureForma may update these Terms as our website, products, services, business structure, or legal requirements change.

The updated version will be posted on the website with a revised “Last Updated” date.

Changes become effective when posted unless another effective date is stated.

Your continued use of the website after updated Terms become effective constitutes acceptance of the revised Terms.
 

34. GOVERNING LAW

These Terms are governed by the laws of the State of California, without regard to conflict-of-law principles.

Unless applicable law requires otherwise or the parties agree otherwise in writing, disputes arising from these Terms will be brought in a court of competent jurisdiction serving Stanislaus County, California.

35. SEVERABILITY

If any portion of these Terms is determined to be invalid, unlawful, or unenforceable, that provision will be enforced to the maximum extent permitted by law and the remaining provisions will continue in effect.

36. NO WAIVER

Failure by PureForma to enforce any provision of these Terms on one occasion does not waive our right to enforce that provision or any other provision in the future.
 

37. ENTIRE AGREEMENT

These Terms, together with any applicable Privacy Policy, Refund Policy, Shipping Policy, booking terms, program rules, service consent forms, or separate agreements, constitute the agreement governing your use of the applicable PureForma product, service, or program.

Where a specific written agreement or service-specific policy conflicts with these general Terms, the more specific agreement or policy controls with respect to that transaction or activity.
